In consideration of you accepting my participation as a Team Achilles volunteer, I the undersigned, intending to be legally bound, hereby waive for myself, my heirs, executors and administrators, any and all rights and claims for damages that I may have against the Achilles Track Club and all participating program sponsors, their officers and representatives, successors and assignees, for any and all injuries or damages suffered by me. I attest and verify that I will participate voluntarily, that I am physically fit, and that my physical condition has been verified by a licensed medical doctor. Furthermore, I hereby grant full permission for any photographs, video-tapes or any other record of this event to be used for any legitimate purpose. |
